Indications
- Treatment of hypertension
- Renal protection in type-2 diabetic patients with proteinuria
Pharmacology
- First non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- For inadequate antihypertensive effect, 25 mg twice daily is recommended
- Total daily dose ranges from 25 mg to 100 mg
- Different dosage for patients upto 75 years and over 75 years
Interaction
- Reduced levels with rifampicin and fluconazole
- Potentiation with concomitant use of hydrochlorothiazide
- Possible increases in serum potassium with potassium-sparing diuretics or supplements
Contraindications
- Contradicted in pregnant women and patients hypersensitive to any component
- Not to be used with aliskiren in diabetic patients
Side Effects
- Mild and transient side effects including dizziness, diarrhea, and nasal congestion
- Other side effects include fatigue, oedema, and abdominal pain
Pregnancy & Lactation
- Pregnancy category D
- Risk to fetus increases if administered during second or third trimesters
- Not known whether excreted in human milk
Precautions & Warnings
- Reduced fetal renal function and increased fetal and neonatal morbidity and death when used in second and third trimesters of pregnancy
- Symptomatic hypotension may occur in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
Use in Special Populations
- Different dosage for renal impairment and hepatic impairment
- Use in patients with intravascular volume depletion
