Indications
- Treatment of hypertension
- Renal protection in type-2 diabetic patients with proteinuria
Pharmacology
- First non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
- Reduces vasoconstriction and aldosterone release
- Responsible for hypertension
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- 25 mg twice daily for inadequate antihypertensive effect
- Total daily dose ranges from 25 mg to 100 mg
- Different starting dose for patients with intravascular volume-depletion
- Usual dose for patients upto 75 years
- Lower starting dose for patients over 75 years
Interaction
- Reduction of active metabolite levels with rifampicin and fluconazole
- Potentiation of antihypertensive effects with hydrochlorothiazide
- Increased serum potassium with potassium-sparing diuretics, supplements, or salt substitutes
- Attenuation of antihypertensive effect by indomethacin
- Risk of renal impairment with certain drug combinations
Contraindications
- Contradicted in pregnant women
- Hypersensitivity to any component of the product
- Contraindicated with Aliskiren in diabetic patients
Side Effects
- Mild and transient side effects
- Common side effects like dizziness, diarrhea, and nasal congestion
- Other side effects like fatigue, oedema, and abdominal pain
Pregnancy & Lactation
- Pregnancy Category D
- Increased risk to fetus in second or third trimester
- Consideration for discontinuing nursing if drug administered
Precautions & Warnings
- Reduced fetal renal function and increased morbidity in second and third trimesters
- Symptomatic hypotension in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
- Changes in renal function and renal failure in renal impaired patients
Use in Special Populations
- No initial dosage adjustment in mild renal impairment
- Lower starting dose for moderate to severe renal impairment or patients on dialysis
- Special dosing consideration for patients with intravascular volume depletion
- Consideration for lower dose in patients with hepatic impairment
