Indications

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Specific inhibition of H+/K+-ATPase in the gastric parietal cell
  • Enteric-coated pellet formulation of esomeprazole magnesium
  • Metabolized in the liver by the cytochrome P450 enzyme system
  • Extensively metabolized by CYP2C19 and CYP3A4
  • Peak plasma levels occur at approximately 1.5 hours after oral administration
  • Plasma protein binding is constant over the concentration range of 2-20 mmol/L

Dosage

  • Various dosage forms including oral capsules, oral suspension, IV injection, and IV infusion

Administration

Interaction

  • Symptomatic response to therapy does not preclude the presence of gastric malignancy
  • Guidelines for administration in patients who have difficulty swallowing capsules
  • Use of antacids while taking the medication

Use in Special Populations

  • Safety and effectiveness in pediatric patients have not been established
  • No overall differences in safety and efficacy observed between the elderly and younger individuals
  • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
  • Pharmacokinetics not expected to be altered in patients with renal impairment

Overdose Effects

  • Signs of acute toxicity observed in rats at high doses
  • No specific antidote known
  • Treatment should be symptomatic and supportive
