Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • Broad-spectrum parenteral cephalosporin antibiotic
  • Potent bactericidal activity against wide range of Gram-positive and Gram-negative organisms
  • Interferes with synthesis of bacterial cell wall
  • High stability in presence of beta lactamases
  • Relatively long plasma elimination half-life of 6 to 9 hours
  • Not metabolized in the body
  • About 40-65% excreted unchanged in the urine

Dosage

  • Adult dose: 1 to 2 gm by intravenous or intramuscular administration once a day
  • Maximum adult dose: 4 gm/day
  • Infants and Children dose: 50 to 75 mg/kg intravenous or intramuscular administration once a day
  • Maximum infants and children dose: 2 gm/day
  • Duration of therapy: 4 to 14 days; longer therapy may be required for complicated infections

Administration

  • Intramuscular / Intravenous Injection preparation instructions
  • Administration over 2-4 minutes for injection
  • Patient tolerance test recommended before treatment
  • Reconstituted solution maintains potency for at least 6 hours at room temperature or 24 hours at 5°C

  • No reported drug interactions

Contraindications

  • Not for patients with history of hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects including diarrhea, nausea, vomiting, stomatitis, and glossitis
  • Cutaneous reactions including rash, pruritus, urticaria, edema, and erythema multiforme
  • Hematologic reactions including eosinophilia, thrombocytopenia, leucopenia, anemia, and neutropenia
  • Hepatic reactions including elevations of SGOT/SGPT, bilirubinemia
  • CNS reactions including nervousness, confusion, sleep disturbances, headache, convulsion, and dizziness
  • Rare occurrence of local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety in human pregnancy not established
  • Caution with lactating mother

Precautions & Warnings

  • Risk of anaphylactic shock
  • Possible gallbladder sludge detection
  • Recommended blood picture check during prolonged treatment

Use in Special Populations

  • Not for neonates if premature and newborn (up to 28 days of age)

Overdose Effects

  • No specific antidote; symptomatic treatment
