Dosage & Administration
- Should be taken 15-30 minutes before meals
- Maximum dose for adults is 80 mg daily
- Maximum period of treatment for acute nausea and vomiting is 12 weeks
Interaction
- Concomitant administration of anticholinergic drugs may antagonize the effect
- Antacids and antisecretory drugs should not be given simultaneously
- Main metabolic pathway is through CYP3A4
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Extrapyramidal phenomena
- Increase in plasma prolactin level
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Use during first trimester justified by therapeutic benefit
- Excreted in breast milk at 4 times lower concentrations than in plasma
- Nursing not recommended unless benefits outweigh potential risk
Precautions & Warnings
- Absolute caution in the case of children
- Increased risk of extra-pyramidal reactions in young children
- Use with caution in patients with hepatic impairment
Use in Special Populations
- Great caution and close medical supervision for infants under 1 year of age
- Caution in patients with hepatic impairment
- Dosing frequency should be reduced in patients with severe renal insufficiency
Overdose Effects
- Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
- Administration of activated charcoal and close observation recommended
- Anticholinergic drugs may be helpful in controlling extrapyramidal reactions
