Indications

  • Hypertension
  • Renal Protection in Type-2 Diabetic Patients with Proteinuria

Pharmacology

  • Non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptor
  • Reduces vasoconstriction
  • Reduces aldosterone release

Dosage & Administration

  • Usual starting and maintenance dose of 50mg once daily
  • May be used alone or in combination with other antihypertensive agents
  • Total daily dose ranges from 25mg to 100mg
  • Different dosage for patients under 75 years and over 75 years

Interaction

  • Rifampicin and fluconazole reduce levels of active metabolite
  • Concomitant use of Losart and hydrochlorothiazide may lead to potentiation of antihypertensive effects
  • Concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes may lead to increase in serum potassium
  • Attenuation by non-steroidal anti-inflammatory drug indomethacin

Contraindications

  • Contraindicated in pregnant women
  • Contraindicated in patients hypersensitive to any component
  • Should not be administered with Aliskiren in patients with diabetes

Pregnancy & Lactation

  • Pregnancy Category D
  • Risk to fetus increases if administered during second or third trimesters
  • Not known whether excreted in human milk

Precautions & Warnings

  • Reduces fetal renal function and increases fetal and neonatal morbidity and death during second and third trimesters of pregnancy
  • Symptomatic hypotension may occur in intravascularly volume-depleted patients
  • Plasma concentration significantly increased in cirrhotic patients
  • Renal failure reported in renal-impaired patients

Use in Special Populations

  • No initial dosage adjustment necessary in patients with mild renal impairment
  • For patients with moderate to severe renal impairment, a lower starting dose is recommended
  • Use in patients with intravascular volume depletion
  • Use in hepatic impairment
  • Lower dose should be considered for patients with a history of hepatic impairment
