Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- Angiotensin II receptor blocker
- Reduces vasoconstriction
- Binds to AT1 receptor
- Orally active
- Reduces release of aldosterone
Dosage & Administration
- Usual starting and maintenance dose is 50 mg once daily
- Can be administered once or twice daily
- Total daily dose ranges from 25 mg to 100 mg
- Adjustment for patients over 75 years
Interaction
- Rifampicin and fluconazole reduce levels of active metabolite
- Concomitant use with hydrochlorothiazide
- Concomitant use with potassium-sparing diuretics leads to increase in serum potassium
- Attenuation by non-steroidal anti-inflammatory drug indomethacin
- Increased risk of renal impairment with certain drug combinations
Contraindications
- Pregnant women
- Hypersensitivity to any component
- Not to be administered with Aliskiren in patients with diabetes

Pregnancy & Lactation

- Risk to the fetus if administered during the second or third trimesters

Precautions & Warnings
- Reduces fetal renal function in second and third trimesters of pregnancy
- Potential symptomatic hypotension in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
- Changes in renal function including renal failure reported in renal impaired patients
Use in Special Populations
- No initial dosage adjustment necessary in patients with mild renal impairment
- Lower starting dose for patients with moderate to severe renal impairment or on dialysis
- Lower dose for patients with a history of hepatic impairment
- Use in patients with intravascular volume depletion
- Contraindications and warnings
- Can be administered with other antihypertensive agents
- Contraindications: pregnancy and hypersensitivity to losartan
- Effect of other drugs on renin-angiotensin-aldosterone system
