  • Governed by desired blood glucose level
  • Lowest effective dosage for desired metabolic control
  • Regular measurement of blood and urine glucose levels
  • Avoidance of larger doses to correct mistakes
  • Potential reduction of glimepiride requirements as treatment proceeds

Administration

  • Must be swallowed whole
  • Not to be crushed or chewed

Interaction

  • Metabolized by cytochrome P450 2C9 (CYP2C9)
  • Potentiation of blood-glucose-lowering effect with insulin and other oral antidiabetics
  • Weakening of blood-glucose-lowering effect with corticosteroids and diazoxide
  • Alcohol intake may influence blood-glucose-lowering action
  • Bile acid sequestrant (e.g., colesevelam) reduces glimepiride absorption

  • Increased risk of hypoglycemia during illness or lifestyle changes
  • Caution in patients with G6PD-deficiency for glimepiride
  • Regular monitoring of thyroid-stimulating hormone (TSH) levels for metformin

Use in Special Populations

  • Insufficient data for pediatric use
  • Assessment of GFR before initiation and at least annual monitoring for renal impairment

Overdose Effects

  • Severe life-threatening hypoglycemia for glimepiride overdose
  • Potential lactic acidosis for high metformin doses
