Indications

Dosage & Administration

  • Recommended adult dose of 80mg orally once daily
  • Consider starting dose of 40mg for patients on high diuretic doses
  • Additional blood pressure reduction with concomitant use of other antihypertensive agents

Interactions

  • No interactions with amlodipine, antacids, chlorthalidone, digoxin, and more
  • Potential attenuation by non-steroidal anti-inflammatory drugs

Contraindications

  • Co-administration contraindicated with aliskiren in diabetes patients

Side Effects

  • Common adverse reaction of diarrhea
  • Other side effects include nausea, asthenia, fatigue, muscle spasm, dizziness, and cough

Pregnancy & Lactation

  • Pregnancy Category D
  • Risk to fetus in second or third trimester
  • Unknown excretion in human milk
  • Consideration for discontinuation of nursing

Precautions & Warnings

  • Use during second and third trimesters reduces fetal renal function
  • Potential symptomatic hypotension in volume-depleted patients
  • Possibility of changes in renal function including renal failure in renal impaired patients

Use in Special Populations

  • Neonatal reaction to in utero exposure
  • No established safety and effectiveness in pediatric patients
  • No dose adjustment for elderly patients
  • Likelihood of abnormally high serum creatinine values in elderly patients
  • No dose adjustment in mild-to-severe renal impairment or end-stage renal disease patients
  • Abnormally high serum creatinine values likely in moderate to severe renal impairment
  • No dose adjustment for mild or moderate hepatic impairment
  • Not studied in severe hepatic impairment

Overdose Effects

  • Limited data related to overdose in humans
  • Supportive therapy in case of overdose
  • Not dialyzable
