Composition

  • Thiopental 500 mg injection
  • Thiopental 1 gm injection
  • Sterile mixture of Thiopental Sodium and anhydrous Sodium Carbonate
  • Ampoules of Water for Injection

Pharmacology

  • Ultrashort-acting depressant of the central nervous system
  • Induces hypnosis and anesthesia
  • Binds at a distinct binding site associated with a Cl- ionopore at the GABAA receptor
  • Increases duration of time for which the Cl- ionopore is open
  • Prolongs the post-synaptic inhibitory effect of GABA in the thalamus

Dosage & Administration

  • Administered intravenously as a 2.5% w/v solution
  • Can be administered as a 5% w/v solution
  • Premedication with atropine or scopolamine
  • Test dose for tolerance assessment
  • Normal dosage for induction of anesthesia
  • Dosage adjustments based on patient's response
  • Specific dosages for adults, children, and elderly
  • Dosage for convulsive states

Interaction

  • Increase in difficulty in producing anesthesia with alcohol or CNS depressants
  • Additive action with other CNS depressants
  • Increased hypotension and excitatory effects with phenothiazine antipsychotics
  • Increased hypnotic effect with antipsychotic
  • Decreased requirement of TPS with specific drugs

Contraindications

  • Absence of suitable veins for intravenous administration
  • Hypersensitivity to barbiturates
  • Variegate porphyria or acute intermittent porphyria
  • Status asthmaticus
  • Severe cardiovascular disease
  • Hypotension or shock
  • Conditions prolonging or potentiating the hypnotic effect
