Indications
- Treatment of hypertension
- Renal protection in type-2 diabetic patients with proteinuria
Pharmacology
- Non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptors in various tissues
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose of 50 mg
- 25 mg twice daily for inadequate antihypertensive effect
- Total daily dose ranges from 25 mg to 100 mg
- Different dosage for patients with intravascular volume-depletion or over 75 years
Interaction
- Reduction in active metabolite levels with rifampicin and fluconazole
- Potentiation of antihypertensive effects with hydrochlorothiazide
- Increase in serum potassium with potassium-sparing diuretics or supplements
- Attenuation of antihypertensive effect by indomethacin
- Increased risk of renal impairment with certain drug combinations
Contraindications
- Contraindicated in pregnant women
- Hypersensitivity to any component
- Not to be administered with Aliskiren in patients with diabetes
Side Effects
- Mild and transient side effects
- Common side effects: dizziness, diarrhea, nasal congestion, cough, upper respiratory infection
- Other side effects: fatigue, edema, abdominal pain, chest pain, nausea, headache, pharyngitis
Pregnancy & Lactation
- Pregnancy Category D
- Potential excretion in human milk
- Risks during second or third trimesters
- Consideration for nursing mothers
Precautions & Warnings
- Reduction in fetal renal function and increased morbidity in late pregnancy
- Symptomatic hypotension in intravascularly volume-depleted patients
- Significant increase in plasma concentration in cirrhotic patients
- Reported changes in renal function and renal failure in renal impaired patients
Use in Special Populations
- No initial dosage adjustment in patients with mild renal impairment
- Lower starting dose for moderate to severe renal impairment or patients on dialysis
- Consideration for patients with hepatic impairment
- Possible effects on blood urea and serum creatinine in certain conditions
