Emestop 125 mg (Capsule)

Indications

• Prevention of postoperative nausea and vomiting (PONV)
• Prevention of Chemotherapy Induced Nausea and Vomiting (CINV)

Pharmacology

• Aprepitant is a selective high affinity antagonist of human substance P neurokinin 1 (NK1) receptors
• Blocks substance P from binding to the NK1 receptors to prevent nausea and vomiting after chemotherapy or surgery

Dosage

• Oral dosage of Aprepitant is 40 mg within 3 hours prior to induction of anesthesia for PONV
• Different regimens for highly emetogenic and moderately emetogenic cancer chemotherapy

Administration

• Aprepitant may be taken with or without food
• No dosage adjustment necessary for elderly patients

Interaction

• Emestop is a substrate, inhibitor, and inducer of CYP3A4 and an inducer of CYP2C9
• Precautions needed when coadministering with drugs utilizing CYP3A4 or CYP2C9
• Reduced efficacy of hormonal contraceptives during and following last dose of Emestop

Contraindications

• Contraindicated in patients hypersensitive to any component
• Should not be used concurrently with specific drugs like Pimozide and Terfenadine

• Pruritus
• Pyrexia

Pregnancy & Lactation

• Pregnancy Category B
• Drug use during pregnancy only if clearly needed
• Decision to discontinue nursing or drug based on patient’s importance

Use in Special Populations

• No dosage adjustment for patients with renal impairment or undergoing hemodialysis
• No dosage adjustment for patients with mild to moderate hepatic impairment

Overdose Effects

• No specific information available on treatment of overdosage
• Single doses up to 600 mg generally well tolerated in healthy subjects
• Drowsiness and headache can occur due to overdose
