Indications
- Treatment of ALK-positive non-small cell lung cancer (NSCLC)
Pharmacology
- Highly selective ALK and RET tyrosine kinase inhibitor
- Induces tumor cell death (apoptosis)
- Distribution into the central nervous system
Dosage
- 600 mg oral dose twice daily
- 450 mg oral dose for patients with severe hepatic impairment
Administration
- Treatment until disease progression
- Dose modification based on tolerability
Interaction
- Weak inhibition of CYP3A4
- No influence on exposure of midazolam
- Weak induction potential of CYP3A4 and CYP2B6 at clinical concentrations
Contraindications
- Known hypersensitivity to alectinib or any excipients

- Immediate interruption in diagnosed ILD/pneumonitis cases
- Monitoring for hepatotoxicity
- Permanent discontinuation in case of no identified causes of ILD/pneumonitis
Use in Special Populations
- Studied safety and efficacy in children and adolescents
- No dose adjustment in patients ≥65 years of age
- No dose adjustment in patients with mild or moderate renal impairment
- No dose adjustment in patients with severe renal impairment for negligible kidney elimination
- Dose adjustment for patients with severe hepatic impairment
