Indications

  • Hypertension
  • Renal Protection in Type-2 Diabetic Patients with Proteinuria

Pharmacology

  • Non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptor in various tissues
  • Reduces vasoconstriction and aldosterone release

Dosage & Administration

  • Usual starting and maintenance dose of 50 mg once daily
  • 25 mg twice daily for inadequate antihypertensive effect
  • Total daily dose ranges from 25 mg to 100 mg
  • Different starting dose for patients with intravascular volume-depletion
  • Lower starting dose for patients over 75 years

Interaction

  • Reduction of active metabolite levels by Rifampicin and fluconazole
  • Potentiation of antihypertensive effects with hydrochlorothiazide
  • Increase in serum potassium with potassium-sparing diuretics
  • Attenuation of antihypertensive effect by indomethacin
  • Increased risk of renal impairment with certain drug combinations

Contraindications

  • Pregnant women
  • Hypersensitivity to any component
  • Concurrent use with Aliskiren in patients with diabetes

Side Effects

  • Mild and transient nature
  • Common side effects: dizziness, diarrhea, nasal congestion, cough, upper respiratory infection
  • Other side effects: fatigue, edema, abdominal pain, chest pain, nausea, headache, pharyngitis

Pregnancy & Lactation

  • Pregnancy Category D
  • Risk to fetus if administered during second or third trimesters
  • Potential excretion in human milk
  • Consideration for discontinuation of nursing or drug based on importance to the mother

Precautions & Warnings

  • Reduced fetal renal function and increased fetal and neonatal morbidity and death during second and third trimesters of pregnancy
  • Symptomatic hypotension in intravascularly volume-depleted patients
  • Significantly increased plasma concentration in cirrhotic patients
  • Changes in renal function including renal failure in patients with renal impairment

Use in Special Populations

  • No initial dosage adjustment for patients with mild renal impairment
  • Lower starting dose for patients with moderate to severe renal impairment or on dialysis
  • Consideration of lower dose for patients with a history of hepatic impairment
