Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
Anti-inflammatory, antipruritic, vasoconstrictive properties
Dosage
- Cream, Ointment:
- Apply a thin layer twice daily
- Limit treatment to 2 consecutive weeks
- Maximum weekly dose: 50 gm/week for adults
- Limited use in children
- Spray: Apply once or twice daily; max weekly dose: 50 ml
- Shampoo: Apply once a day; max consecutive use: 4 weeks; total dosage not to exceed 50 g per week
- Scalp Solution: Apply once or twice daily; max weekly dose: 50 ml
Administration
Cutaneous; creams for moist or weeping surfaces, ointments for dry, lichenified, or scaly lesions
Interaction
CYP3A4 inhibition can lead to increased systemic exposure
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Primary cutaneous viral infections
- Hypersensitivity to the preparation
- Not indicated for primarily infected skin lesions
- Dermatoses in children under one year of age
Side Effects
- Hypercorticism with prolonged use
- Local atrophic changes with intensive treatment
- Possible provocation of pustular form of psoriasis
- Pigmentation changes and hypertrichosis
- Signs of hypersensitivity may occur
Pregnancy & Lactation
- Limited data in pregnant women
- Potential risks; benefit should outweigh risks
- Caution during nursing
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Atrophic changes on the face after prolonged treatment
- Careful application to eye lids to prevent eye entry
- Hazardous in psoriasis with risk of relapses, tolerance, toxicity
- Appropriate anti-microbial therapy for infected lesions
Use in Special Populations
Avoid long-term continuous therapy in infants and children
Overdose Effects
Chronic overdosage may lead to hypercortisolism
