Medicine Details

Indications

  • Treatment of hypertension
  • Can be used alone or in combination with other antihypertensive agents

Pharmacology

  • Formed from angiotensin I in a reaction catalyzed by angiotensin converting enzyme
  • Principal pressor agent of the renin-angiotensin system
  • Selective blocking of the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
  • More than 12,500-fold greater affinity for the AT 1 receptor than for the AT 2 receptor
  • Does not inhibit ACE (kininase II), does not affect the response to bradykinin
  • Inhibits the negative regulatory feedback of angiotensin II on renin secretion

Dosage & Administration

  • Usual recommended starting dose of 20 mg once daily as monotherapy
  • For patients requiring further reduction in blood pressure, the dose may be increased to 40 mg
  • No initial dosage adjustment recommended for elderly patients
  • Consideration should be given to use of a lower starting dose for patients with possible depletion of intravascular volume
  • May be administered with or without food
  • Safety and effectiveness in pediatric patients have not been established

Interaction

  • No significant drug interactions with digoxin or warfarin
  • Bioavailability not significantly altered by the co-administration of antacids
  • Not metabolized by the cytochrome P450 system and has no effects on P450 enzymes
  • Co-administration with NSAIDs including selective COX-2 inhibitors may result in deterioration of renal function
  • Antihypertensive effect may be attenuated by NSAIDs including selective COX-2 inhibitors

Contraindications

  • Hypersensitivity to any component of this product

Side Effects

  • Well tolerated with an incidence of adverse reactions similar to placebo
  • Overall frequency of adverse reactions not dose related
  • Rate of withdrawals due to adverse reactions was low
  • Most common adverse reaction was dizziness
  • Other adverse reactions include back pain, bronchitis, diarrhea, headache, hyperglycemia, pharyngitis, and sinusitis

Pregnancy & Lactation

  • Discontinue the product when pregnancy is detected
  • Can cause injury and even death to the developing fetus when used during the second and third trimesters
  • Potential for adverse effects on the nursing infant

Precautions & Warnings

  • Changes in renal function may be anticipated in susceptible individuals
  • Treatment has been associated with oliguria, progressive azotemia, acute renal failure, and death in some patients

Overdose Effects

  • No experience of overdose with the product
  • Most likely effects of overdosage are hypotension and tachycardia
  • Clinically significant hypotension requires active support of the cardiovascular system
