Indications
- Treatment of hypertension
- Can be used alone or in combination with other antihypertensive agents
Pharmacology
- Formed from angiotensin I in a reaction catalyzed by angiotensin converting enzyme
- Principal pressor agent of the renin-angiotensin system
- Causes vasoconstriction, stimulation of synthesis and release of aldosterone, cardiac stimulation, and renal reabsorption of sodium
- Blocks vasoconstrictor effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
- Has more than a 12,500-fold greater affinity for the AT 1 receptor than for the AT 2 receptor
- Does not inhibit ACE (kininase II) or affect the response to bradykinin
- Inhibits the negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily when used as monotherapy
- For patients requiring further reduction in blood pressure, the dose may be increased to 40 mg
- No advantage in twice-daily dosing over the same total dose given once daily
- No initial dosage adjustment recommended for elderly patients or for patients with moderate to marked renal impairment or hepatic dysfunction
- May be administered with or without food
- Safety and effectiveness in pediatric patients not established
Interaction
- No significant drug interactions reported for digoxin or warfarin in healthy volunteers
- Bioavailability not significantly altered by the co-administration of antacids
- Not metabolized by the cytochrome P450 system and has no effects on P450 enzymes
- Possible deterioration of renal function when co-administered with NSAIDs, including selective COX-2 inhibitors
Contraindications
- Contraindicated in patients who are hypersensitive to any component of the product
Side Effects
- Evaluated for safety in more than 3825 patients
- Incidence of adverse reactions similar to placebo
- Overall frequency of adverse reactions not dose related
- The rate of withdrawals due to adverse reactions was 2.4% of patients treated with the product
- Most common adverse reaction: dizziness
- Other adverse reactions occurred at an incidence of more than 1% of patients, but also occurred at similar or greater incidence in patients receiving placebo
Pregnancy & Lactation
- Discontinue the product as soon as pregnancy is detected
- Drugs that act directly on the renin-angiotensin system can cause injury and death to the developing fetus
- Potential excretion in human milk, decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother
Precautions & Warnings
- Changes in renal function may be anticipated in susceptible individuals
- Treatment has been associated with oliguria, progressive azotemia, acute renal failure, and death especially in patients with severe congestive heart failure
Overdose Effects
- No experience of overdose reported
- Most likely effects of overdose: hypotension and tachycardia
- Gastric lavage or induction of emesis may be considered if intake is recent
- Clinically significant hypotension due to overdose requires active support of the cardiovascular system
