Description
- Olmecar is an angiotensin II receptor blocker used for the treatment of hypertension.
- It can be used alone or in combination with other antihypertensive agents.
- It blocks the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle.
- The usual recommended starting dose is 20 mg once daily, which may be increased to 40 mg if necessary.
- It is generally well tolerated with an overall frequency of adverse reactions similar to placebo.
- It is contraindicated in patients who are hypersensitive to any component of the product.
- Discontinue the product if pregnancy is detected as it can cause injury to the developing fetus.
Dosage & Administration
- Individualized dosage
- Starting dose of 20 mg once daily
- Further reduction of blood pressure may require an increased dose to 40 mg
- Twice-daily dosing offers no advantage over once daily dosing
- No initial dosage adjustment recommended for elderly patients or those with moderate to marked renal impairment or hepatic dysfunction
- Consideration of lower starting dose for patients with possible depletion of intravascular volume
- May be administered with or without food
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by co-administration of antacids
- Not metabolized by the cytochrome P450 system, and has no effects on P450 enzymes
- Co-administration with NSAIDs may result in deterioration of renal function
- Attenuation of the antihypertensive effect by NSAIDs including selective COX-2 inhibitors

- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks the vasoconstrictor effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
- Does not inhibit ACE (kininase II) or affect the response to bradykinin
- Inhibits the negative regulatory feedback of angiotensin II on renin secretion
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with an incidence of adverse reactions similar to placebo
- Rate of withdrawals due to adverse reactions was low
- Dizziness was the most common adverse reaction
- Other adverse reactions occurred at similar or greater incidence in patients receiving placebo
Pregnancy & Lactation
- Discontinue the product when pregnancy is detected
- Can cause injury and even death to the developing fetus during the second and third trimesters
- Potential for adverse effects on the nursing infant
Precautions & Warnings
- Changes in renal function may be anticipated
- Treatment associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- No experience of overdose reported
- Likely effects include hypotension and tachycardia
- Clinically significant hypotension requires active support of the cardiovascular system
