Indications

  • Treatment of hypertension
  • Reducing risk of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Losartan is a potent vasoconstrictor
  • Hydrochlorothiazide is a thiazide diuretic affecting renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Usual starting dose is one tablet once daily
  • Maximum dose is 100/25 once daily
  • Antihypertensive effect is attained within three weeks
  • No initial dosage adjustment for elderly patients

Administration

  • May be administered with or without food

Interaction

  • No significant drug-drug pharmacokinetic interactions with certain drugs
  • Concomitant use of potassium-sparing diuretics may lead to increase in serum potassium

Contraindications

  • Contraindicated in patients hypersensitive to any component
  • Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness may occur
  • Hyperkalaemia occurs occasionally
  • Vertigo, gastro-intestinal disturbances, angina, palpitation, oedema, dyspnoea, headache, sleep disorders, malaise, urticaria, pruritus, rash reported

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • Not recommended in breastfeeding
  • Alternative treatment options available

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Hypokalemia may rarely develop especially with brisk diuresis
  • Impaired renal function and symptomatic hypotension to be considered

Use in Special Populations

  • Usual regimens of therapy may be followed with creatinine clearance >30 ml/min
  • Loop diuretics preferred in patients with severe renal impairment

Overdose Effects

  • Manifestations of overdosage could include hypotension, tachycardia, and bradycardia
  • Supportive treatment should be instituted if symptomatic hypotension occurs
