Indications
- Treatment of hypertension
- Reducing risk of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Blockage of vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Selective binding of angiotensin II to the AT<sub>1 </sub>receptor
- Potent vasoconstrictor and primary vasoactive hormone
- Stimulation of aldosterone secretion
- Thiazide diuretic effect on renal tubular mechanisms
Dosage
- Usual starting dose of 50/12.5: one tablet once daily
- Antihypertensive effect attained within three weeks
- No initial dosage adjustment necessary for elderly patients
Administration
- May be administered with other antihypertensive agents
- May be administered with or without food
Interaction
- No significant drug-drug pharmacokinetic interactions found in interaction studies
- Increased serum potassium with concomitant use of potassium-sparing diuretics
- Potential interaction with non-steroidal anti-inflammatory drug Indomethacin
- Possible interaction with alcohol, barbiturates, narcotics
- Possible dosage adjustment of antidiabetic drugs
- Additive effect or potentiation with other antihypertensive drugs
Contraindications
- Hypersensitivity to any component of the product
- Anuria or hypersensitivity to other sulfonamide-derived drugs
Side Effects
- Mild side effects such as symptomatic hypotension, dizziness, hyperkalaemia, angioedema
- Vertigo, gastro-intestinal disturbances, angina, palpitation, oedema, dyspnoea, headache, sleep disorders
- Malaise, urticaria, pruritus, rash, hepatitis, atrial fibrillation, cerebrovascular accident, syncope
- Pancreatitis, anaphylaxis, cough, depression, erectile dysfunction, anaemia, thrombocytopenia
- Hyponatraemia, arthralgia, myalgia, renal impairment, rhabdomyolysis, tinnitus, photosensitivity, vasculitis
Pregnancy & Lactation
- Avoid in pregnancy unless essential
- Adverse effects on fetal and neonatal blood pressure control and renal function
- Not recommended in breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes
- Risk of hypokalemia especially with brisk diuresis or severe cirrhosis
- Risk of impaired renal function and symptomatic hypotension
Use in Special Populations
- Usual regimens of therapy may be followed with creatinine clearance >30 ml/min
- Loop diuretics preferred in more severe renal impairment
- Not recommended in patients with hepatic impairment
- Safety and effectiveness not established in pediatric patients
Overdose Effects
- Manifestations of overdosage may include hypotension, tachycardia, bradycardia
- Supportive treatment for symptomatic hypotension
- No removal by hemodialysis
