Indications

  • Lower respiratory tract infections
  • Acute Bacterial Otitis Media
  • Skin and skin structure infections
  • Urinary tract infections
  • Gonorrhea
  • Bacterial Septicemia
  • Bone and joint infections
  • Meningitis
  • Prevention of postoperative infections
  • Perioperative prophylaxis of infections associated with surgery

Pharmacology

  • 3rd generation broad-spectrum parenteral cephalosporin antibiotic
  • Bactericidal activity against Gram-positive and Gram-negative organisms
  • Interferes with bacterial cell wall synthesis
  • High stability in the presence of beta lactamases
  • Long plasma elimination half-life of 6 to 9 hours
  • Not metabolized in the body
  • Highly protein bound (95%)

Dosage

  • Adult dosage: 1 to 2 gm intravenously or intramuscularly once a day
  • Maximum adult dose: 4 gm/day
  • Infants and Children dosage: 50 to 75 mg/kg intravenously or intramuscularly once a day
  • Maximum pediatric dose: 2 gm/day
  • Duration of therapy: 4 to 14 days

Administration

  • Preparation of solutions for intramuscular/ intravenous injections
  • Administered over 2-4 minutes intramuscularly or intravenously
  • Freshly reconstituted solution recommended
  • Room temperature stability of 6 hours or 24 hours at 5°C

  • No reported drug interactions

Contraindications

  • Hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastro-intestinal effects like diarrhea, nausea, vomiting
  • Cutaneous reactions including rash, pruritus, urticaria
  • Hematologic reactions like eosinophilia, thrombocytopenia
  • Hepatic reactions such as elevations of SGOT or SGPT
  • CNS reactions like nervousness, confusion
  • Local phlebitis following intravenous administration

Pregnancy & Lactation

  • Safety not established in human pregnancy
  • Caution advised during lactation

Precautions & Warnings

  • Risk of anaphylactic shock
  • Monitor blood picture during prolonged treatment

Use in Special Populations

  • Not recommended for premature and newborn neonates

Overdose Effects

  • No specific antidote, treat symptomatically
