Pivcilin 200 mg (Tablet)

Indications

• Treatment of infections caused by mecillinam-sensitive organisms
• Acute cystitis
• Complicated urinary tract infections
• Salmonellosis
• Shigellosis
• Enteropathic E. coli diarrhoea
• Gram-negative septicaemia
• Billiary infections

Pharmacology

• Belongs to a new class of penicillin-the amdinopenicillin
• Prodrug formulation of mecillinam
• Oral administration
• Bactericidal action
• Interferes with bacterial cell wall synthesis
• Active against Enterobacteriaceae
• Poor activity against Gram-positive organisms
• Poor activity against Pseudomonas aeruginosa and Enterococcus faecalis

Dosage

• Usual dose for adults is 1-2 tablets 3 times daily according to severity of the infection
• Specific dosage for acute uncomplicated cystitis, chronic or recurrent bacteriurea, salmonellosis, and prophylactic treatment of recurrent urinary tract infections
• Dosage for children based on weight and specific conditions such as shigellosis, UTI, and salmonellosis

Administration

• Tablet should be taken with at least 50-100 ml fluid
• Bioavailability is not affected by simultaneous food intake
• Best taken with or immediately after a meal

Contraindications

• Patients with known history of hypersensitivity to penicillins and cephalosporins

Side Effects

• Upper gastrointestinal disturbances such as nausea, vomiting, and diarrhoea
• Skin rashes
• No evidence of hepato-, nephro-, or ototoxicity
• Possibility of anaphylaxis

Pregnancy & Lactation

• Avoid use in pregnancy
• Not excreted into the milk of lactating mother

Precautions & Warnings

• Raised blood levels may occur with impaired renal function

Use in Special Populations

• Dosage adjustment based on renal function
• Avoid use in infants under 3 months of age

Overdose Effects

• Likely to cause nausea, vomiting, and gastritis
• Treatment restricted to symptomatic and supportive measures
