Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective binding to the AT 1 receptor in vascular smooth muscle
- Does not inhibit ACE (kininase II)
- Does not affect the response to bradykinin
- Inhibits the negative regulatory feedback of angiotensin II on renin secretion
Dosage & Administration
- Usual recommended starting dose 20 mg once daily
- May be increased to 40 mg for further reduction in blood pressure
- No initial dosage adjustment for elderly patients or those with moderate to marked renal impairment or hepatic dysfunction
- Lower starting dose for patients with possible depletion of intravascular volume
Interaction
- No significant drug interactions with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Not metabolized by the cytochrome P450 system
- Potential deterioration of renal function with NSAIDs
- Attenuation of antihypertensive effect by NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with no dose-related adverse reactions
- Incidence of adverse reactions similar to placebo
- Most common adverse reaction: dizziness
- Other adverse reactions include back pain, diarrhea, headache, and hypertriglyceridemia
Pregnancy & Lactation
- Discontinue use when pregnancy is detected
- Drugs may cause injury or death to the developing fetus
- Potential secretion in human milk
Precautions & Warnings
- May lead to changes in renal function
- Anticipated results in patients with severe congestive heart failure

- Likely effects: hypotension and tachycardia
