Indications
- Essential hypertension

Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Influences myocardial cells
- Affects specialized conducting system of the heart
- Affects cells of vascular smooth muscle
Dosage & Administration
- Usual dose of 5 mg once daily for hypertension
- Maximum dose of 10 mg once daily for hypertension
- Lower dose for elderly and patients with hepatic insufficiency for angina
- Most patients require 10 mg for angina
- Can be taken without regard to meals
Interaction
- Hazardous interaction with p-blockers in patients with markedly impaired cardiac left ventricular function
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect on prothrombin time
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women

Pregnancy & Lactation
- Pregnancy Category C
- Use only if potential benefit justifies potential risk to the fetus
- Recommendation to discontinue nursing while administered
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breastfeeding
Use in Special Populations
- Starting dose for children with hypertension from 6 years to 17 years of age: 2.5 mg once daily
- Up-titrated to 5 mg once daily for children with hypertension from 6 years to 17 years of age if blood pressure goal is not achieved
- Effect on blood pressure in patients under 6 years of age is not known
- Normal dosage regimens recommended in the elderly
- Dose selection should start at the lower end in patients with mild to moderate hepatic impairment
- Initiation at the lowest dose (2.5 mg once daily) and slow titration in patients with severe hepatic impairment
Overdose Effects
- Large overdosage could result in excessive peripheral vasodilatation
- Possibly reflex tachycardia
- Marked and prolonged systemic hypotension
- Shock with fatal outcome reported
- Active cardiovascular support required in clinically significant hypotension due to overdose
- Intravenous calcium gluconate may be beneficial in reversing effects of calcium channel blockade
- Gastric lavage may be worthwhile in some cases
- Use of charcoal up to 2 hours after administration has been shown to reduce absorption rate
