Indications

  • Lowering blood glucose in type 2 diabetes mellitus patients
  • Adjunct to diet and exercise
  • Used in combination with Metformin

Description

  • Oral blood-glucose-lowering drug
  • Belongs to the meglitinide class
  • Works by causing pancreas to release more insulin

Pharmacology

  • Binds to specific receptors in cell membrane
  • Leads to closure of ATP dependent K channels
  • Results in depolarisation of cell membrane
  • Leads to Ca influx and stimulated insulin secretion

Dosage & Administration

  • Starting dose of 0.5 mg before each meal for certain patients
  • Initial dose of 1 or 2 mg before each meal for previously treated patients
  • Should be taken immediately or up to 30 minutes before each meal

Interaction

  • Possible interactions with cytochrome P450 enzyme system
  • Interactions with inhibitors and inducers affecting clearance and metabolism
  • No significant effect on Digoxin, Theophyllin, or Warfarin

Contraindications

  • Diabetic ketoacidosis
  • Type 1 diabetes mellitus
  • Known hypersensitivity to the drug or its inactive ingredients

Side Effects

  • Hypoglycemia and related symptoms
  • Upper respiratory tract infections
  • Diarrhea, constipation, nausea, vomiting
  • Hypersensitivity reactions including rashes and urticaria

Pregnancy & Lactation

  • Safety in pregnant women not established
  • Use during pregnancy only if clearly needed
  • Not known whether excreted in human milk

Precautions & Warnings

  • Insulin substitution during concurrent illness or surgery
  • Capable of producing hypoglycemia
  • Administered with meals to lessen the risk of hypoglycemia

Overdose Effects

  • Patients receiving up to 80 mg developed few adverse effects
  • Severe hypoglycemic reactions with coma or seizure infrequent
