Indications

  • Treatment of hypertension

Pharmacology

  • Blocks vasoconstrictor effects of angiotensin II
  • Selective binding to AT 1 receptor in vascular smooth muscle
  • No effect on ACE (kininase II) or bradykinin response

Dosage & Administration

  • Usual starting dose of 20 mg once daily
  • May be increased to 40 mg for further blood pressure reduction
  • No initial dosage adjustment recommended for elderly patients
  • May be administered with or without food
  • No established safety and effectiveness for pediatric use

Interaction

  • No significant drug interactions reported with digoxin or warfarin
  • No effect on cytochrome P450 system or P450 enzymes
  • Potential interaction with NSAIDs and COX-2 inhibitors in elderly or volume-depleted patients

Contraindications

  • Hypersensitivity to any component of the product

Side Effects

  • Well tolerated with adverse reaction incidence similar to placebo
  • Most common adverse reaction is dizziness
  • Other adverse reactions may include back pain, diarrhea, headache, hyperglycemia, etc.

Pregnancy & Lactation

  • Discontinue product as soon as pregnancy is detected
  • Potential for adverse effects on nursing infant may require discontinuation of nursing or drug

Precautions & Warnings

  • May cause changes in renal function in susceptible individuals
  • Association with oliguria, acute renal failure, and death in patients dependent on renin-angiotensin-aldosterone system

Overdose Effects

  • No experience of overdose with Olmezest
  • Likely effects include hypotension and tachycardia
  • Clinically significant hypotension may require active cardiovascular system support
