Indications

  • Management of moderate to severe post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Reduction of eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to pyrrolo-pyrrole group of NSAIDs
  • Inhibits synthesis of prostaglandins
  • Metabolized in the liver
  • Highly protein bound

Pharmacology

  • Inhibits cyclooxygenase enzyme system
  • Demonstrates minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet: Recommended dose is 10 mg every 4-6 hours for up to 7 days
  • Injection: Recommended single dose is 60 mg for patients under 65 years
  • Eye Drops: Recommended dosage is 1 drop in each eye 4 times daily
  • Caution for dosage in patients over 65 years and with renal impairment

Interaction

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE Inhibitors
  • Enhanced toxicity of methotrexate

Contraindications

  • Hypersensitivity to ketorolac or other NSAIDs
  • Use in children under 16 years of age
  • Prophylactic analgesic before surgery

Side Effects

  • Nausea, vomiting, gastrointestinal bleeding, melana, peptic ulcer, pancreatitis
  • Anxiety, drowsiness, headache, excessive thirst, fatigue
  • Cardiac effects: bradycardia, hypertension, palpitation, chest pain
  • Infertility in female, pulmonary edema

  • FDA pregnancy category C
  • Avoid in pregnancy and lactation unless benefits outweigh risks

Precautions & Warnings

  • Caution in patients over 65 years
  • Caution in patients with peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Special caution for patients with corneal epithelial breakdown when using eye drops
