Indications
- Treatment of erectile dysfunction

Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Results in smooth muscle relaxation and inflow of blood into penile tissues
- Requires sexual stimulation for effectiveness
Mode of Action
- Involves release of nitric oxide during sexual stimulation
- Activates guanylate cyclase, leading to increased levels of cGMP
Pharmacology
- Mean absolute bioavailability of 41%
- Eliminated by hepatic metabolism
- Metabolized by cytochrome P450 3A4
- Maximum plasma concentrations reached within 30 to 120 minutes post oral dosing
- Excreted predominantly in feces (80%) and to a lesser extent in urine (13%)
Dosage & Administration
- Recommended dose of 50 mg taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Maximum recommended dosing frequency is once per day
- Recommended dose for pulmonary arterial hypertension: 20 mg three times a day
Interaction
- Metabolism primarily mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
- Inducers of these isoenzymes may increase clearance
Contraindications
- Hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates

- Includes body, cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital effects
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use in women
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Potential for additive effect on blood pressure when combined with vasodilators
- Special caution in patients with preexisting cardiovascular disease
- No controlled clinical data in specific patient groups
Overdose Effects
- Adverse events seen at higher incidence and severity with increased doses
- Standard supportive measures should be adopted in cases of overdose
