Indications
- Treatment of erectile dysfunction in men
- Also indicated for pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation and inflow of blood into the penile tissues
Pharmacology
- Mechanism of action involves release of nitric oxide in corpus cavernosum during sexual stimulation
- Rapidly absorbed after oral administration with a mean absolute bioavailability of 41%
- Metabolized predominantly by hepatic metabolism
- Cleared by hepatic microsomal isoenzymes
- Reduced clearance in elderly volunteers
Dosage & Administration
- Recommended dose of 50 mg taken approximately 1 hour before sexual activity
- Maximum recommended dosing frequency is once per day
- Increased plasma levels associated with age >65, hepatic impairment, severe renal impairment, and concomitant use of potent cytochrome P450 3A4 inhibitors
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use of organic nitrates

- Serious cardiovascular, cerebrovascular, and vascular events reported post-marketing
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
Precautions & Warnings
- Caution advised when PDE5 inhibitors are co-administered with alpha-blockers
- Potential for cardiac risk in patients with preexisting cardiovascular disease
- Sensitivity in patients with certain underlying conditions
- Potential for prolonged erection and priapism
- Caution in patients taking ritonavir
Overdose Effects
- In studies with healthy volunteers of single doses up to 800 mg, adverse events were similar to those seen at lower doses
