Therapeutic Class
- Angiotensin-ll receptor blocker
Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Stimulates synthesis and release of aldosterone
- Has a greater affinity for the AT 1 receptor than the AT 2 receptor
- Does not inhibit ACE (kininase II)
Dosage & Administration
- Usual recommended starting dose is 20 mg once daily
- May be increased to 40 mg for patients requiring further reduction in blood pressure
- No initial dosage adjustment recommended for elderly patients or those with moderate to marked renal impairment
- May be administered with or without food
- No advantage of twice-daily dosing over once daily
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Not metabolized by cytochrome P450 system
- Potential interaction with NSAIDs including selective COX-2 inhibitors
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with an incidence of adverse reactions similar to placebo
- Overall frequency of adverse reactions not dose related
- Common adverse reaction: dizziness
- Other adverse reactions include back pain, diarrhea, headache, hyperglycemia, etc.
Pregnancy & Lactation
- Discontinue product when pregnancy is detected
- Can cause injury and death to the developing fetus during the second and third trimesters
- Potential secretion in the milk of lactating individuals
Precautions & Warnings
- May cause changes in renal function
- Treatment associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- Potential effects: hypotension and tachycardia
- Management includes close monitoring of heart and lung function
