Indications
- Adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus
- Not to be used in patients with type 1 diabetes
- Not effective for the treatment of diabetic ketoacidosis
- Not studied in patients with a history of pancreatitis
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases concentration of active intact hormones
- Regulates glucose homeostasis
- Increases insulin release and decreases glucagon levels
Dosage & Administration
- Recommended dose: 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- No meaningful alteration of pharmacokinetics of various drugs observed
- Slight increase in mean Digoxin concentration
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia may occur in patients treated with combination therapy
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Caution advised during lactation
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment recommended in patients with renal insufficiency
- Caution with medications known to cause hypoglycemia
- Reports of serious hypersensitivity reactions
- Safety and effectiveness not established in pediatric patients under 18 years

- Substantially excreted by the kidney
- Dose selection caution in the elderly
- Dosage adjustments for varying levels of renal insufficiency
Overdose Effects
- Maximal mean increases in QTc observed at high doses
- Usual supportive measures recommended in the event of overdose
- Modestly dialyzable
