Indications

Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus

Pharmacology

Combines sitagliptin and metformin to improve glycemic control in patients with type 2 diabetes

Dosage & Administration

Dose should be individualized based on patient's current regimen and tolerance, generally given twice daily with meals

Interaction

Cationic drugs eliminated by renal tubular secretion: Use with caution
Metformin may decrease the anticoagulant effect of phenprocoumon
Levothyroxine can reduce the hypoglycemic effect of metformin

Contraindications

Contraindicated in patients with renal disease, acute or chronic metabolic acidosis, and history of serious hypersensitivity reaction to the tablet or sitagliptin

Side Effects

Common adverse reactions include diarrhea, upper respiratory tract infection, headache, and hypoglycemia
Adverse reactions due to initiation of metformin therapy include diarrhea, nausea/vomiting, flatulence, and headache

Pregnancy & Lactation

Pregnancy Category B; caution advised when administered to nursing women

Precautions & Warnings

Lactic acidosis risk increases with conditions such as sepsis, dehydration, excess alcohol intake, hepatic/renal impairment, and acute congestive heart failure
Regular monitoring of TSH levels and vitamin B12 levels is recommended
Do not use in patients with hepatic disease or undergoing radiologic studies involving iodinated contrast materials

Overdose Effects

Sitagliptin is modestly dialyzable; prolonged hemodialysis may be considered if clinically appropriate
Metformin is dialyzable with a clearance of up to 170 mL/min under good hemodynamic conditions; hemodialysis may be useful for removal of accumulated drug
