Indications
- Adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus
- Not indicated for type 1 diabetes or diabetic ketoacidosis
- Not studied in patients with a history of pancreatitis
Pharmacology
- Combines sitagliptin and metformin to improve glycemic control in type 2 diabetes
- Mechanism of action of sitagliptin as a DPP-4 inhibitor
- Mechanism of action of metformin as a biguanide class agent
- Involvement of incretin hormones in glucose homeostasis
Dosage & Administration
- Individualized dosing based on patient's current regimen, effectiveness, and tolerability
- Maximum recommended daily dose of 100 mg sitagliptin and 2000 mg metformin
- Twice daily administration with meals
Interaction
- Cationic drugs elimination caution
- Metformin may decrease the anticoagulant effect of phenprocoumon
- Levothyroxine can reduce the hypoglycemic effect of metformin
Contraindications
- Renal disease or dysfunction
- Acute or chronic metabolic acidosis
- History of serious hypersensitivity reaction
Side Effects
- Common adverse reactions such as diarrhea, upper respiratory tract infection, and headache
- Hypoglycemia as an adverse reaction with certain combinations
- Nasopharyngitis reported in patients on sitagliptin monotherapy
Pregnancy & Lactation
- Safety in pregnant women not well-known
- Caution advised for nursing women
