Indications

• Treatment for obese patients with BMI >30 kg/m^2
• Treatment for overweight patients with associated risk factors
• Short-term treatment for adolescents with obesity complications

Pharmacology

• Potent, specific and long-acting lipase inhibitor
• Forms covalent bond with gastric and pancreatic lipases
• Inhibits dietary fat absorption by approximately 30%

Dosage & Administration

• Recommended dose of 120 mg capsule with each main meal
• Effect results in increased fecal fat 24-48 hours after dosing
• Safety & efficacy studied in clinical trials lasting up to 4 years

Interaction

• Reduction in cyclosporine plasma levels when coadministered
• Inhibition of absorption of vitamin E acetate supplement
• Possible decreased absorption of vitamin D, vitamin A, and vitamin K
• Potential impact on patients taking warfarin or antiepileptic drugs

Contraindications

• Patients with chronic malabsorption syndrome
• Patients with cholestasis
• Patients hypersensitive to orlistat or other capsule ingredients

Side Effects

• Common gastrointestinal side effects such as oily spotting, flatulence, and abdominal discomfort
• Rare occurrence of influenza, anxiety, headache, and fatigue
• Rare cases of hypersensitivity with clinical symptoms

Pregnancy & Lactation

• Orlistat contraindicated during pregnancy
• Minimal weight gain or no weight loss recommended for pregnant women
• Caution advised when administered to nursing women

Precautions & Warnings

• Organic causes of obesity should be excluded before prescribing
• Coadministration with cyclosporine should be avoided
• Closely monitor antidiabetic drug treatment during therapy
• Encourage adherence to dietary recommendations and fat distribution
• Strongly encourage multivitamin supplement intake

Overdose Effects

• Single doses of up to 800 mg studied without significant adverse findings
• Significant overdose may require patient observation for 24 hours
