Description

    Pharmacology

    • Mechanism of Action: Inhibition of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
    • Pharmacokinetics and Metabolism:
      • Mean absolute bioavailability of 41%
      • Elimination predominantly by hepatic metabolism
      • Converted to an active metabolite with properties similar to the parent
      • Terminal half lives of about 4 hours
    • Absorption and Distribution:
      • Rapid absorption
      • Maximum observed plasma concentrations reached within 30 to 120 minutes
    • Metabolism and Excretion:
      • Cleared by hepatic microsomal isoenzymes
      • Excreted as metabolites predominantly in the feces
    • Pharmacokinetics in Special Populations: Reduced clearance in healthy elderly volunteers

    Dosage & Administration

    • Recommended dose for erectile dysfunction: 50 mg
    • Recommended dose for pulmonary arterial hypertension: 20 mg three times a day
    • Dosing frequency: once per day

    Interaction

    • Metabolism primarily mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
    • Inhibitors of these isoenzymes may reduce clearance
    • Inducers of these isoenzymes may increase clearance
    • Enhanced plasma levels with inhibitors such as cimetidine, erythromycin, saquinavir, ketoconazole, itraconazole, ritonavir, and bosentan

    • Hypersensitivity to any component
    • Concomitant use with organic nitrates

    Adverse events similar to lower doses, increased incidence rates and severities
