Indications
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease or type 2 diabetes mellitus with documented target organ damage
Pharmacology
- Non-peptide angiotensin-ll receptor antagonist
- Blocks vasoconstrictor and aldosterone secreting effects of angiotensin-ll
- Greater affinity for the AT1 receptor than for the AT2 receptor
Dosage & Administration
- Usual starting dose of 40 mg once a day
- Blood pressure response is dose-related over the range of 20 to 80 mg
- Maximal reduction is generally attained after 4 weeks

Geriatric Use
No overall differences in effectiveness and safety observed
Hepatic Impairment
Monitor carefully and up titrate slowly in patients with biliary obstructive disorders or hepatic insufficiency
Interaction
- Increased risk of renal impairment and loss of antihypertensive effect when co-administered with NSAIDs
- Avoid co-administration with aliskiren in patients with diabetes
Contraindications
Known hypersensitivity to the product or any of its components
Side Effects
- Common side effects in hypertensive patients include sinus pain, congestion, back pain, and diarrhea
- Common side effects in patients of cardiovascular risk reduction include intermittent claudication and skin ulcer
Pregnancy & Lactation
Pregnancy Category C (first trimester) and D (second and third trimester), potential for adverse effects on nursing infant
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Monitor carefully in patients with impaired hepatic or renal function
- Avoid concomitant use of ACE inhibitor and angiotensin receptor blocker
Overdose Effects
Manifestations could include hypotension, dizziness, tachycardia, bradycardia, increase in serum creatinine, and acute renal failure
