Indications
- Rapid iron supply for deficiency
- Treatment for patients unable to tolerate oral iron therapy
- Treatment for patients with active inflammatory bowel disease
- Anemia in chronic kidney disease patients receiving erythropoietin
- Anemia in chronic kidney disease patients not receiving erythropoietin
- Anemia in hemodialysis dependent-chronic kidney disease patients receiving erythropoietin
- Anemia in peritoneal dialysis dependent-chronic kidney disease patients receiving erythropoietin
- Treatment of iron deficiency anemia in surgical patients
- Treatment of iron deficiency anemia in blood donors

Pharmacology
- Therapeutic class: Haematinic
- Composition: Polynuclear Iron (III) Hydroxide in Sucrose
- Sucrose concentration: Approximately 30% w/v (300 mg/ml)
- pH range: 10.5-11.1
- Administration: Intravenous use
- Dissociation and transfer of iron and sucrose by reticuloendothelial system
- Iron distribution in liver, spleen, and bone marrow
- Ferritin binding and sequestering of iron
- Transferrin synthesis and distribution
- Elimination half-life: 6 hours
- Total clearance: 1.2 L/h
- Volume distribution: 10.0 L (non-steady state), 7.9 L (steady state)
Dosage
- Adults and Elderly: 5-10 ml Iron Sucrose Injection (100-200 mg Iron) once to three times a week
- Children: Limited data available; dosage based on clinical need and haemoglobin level
Administration
- Intravenous injection: Can be administered undiluted at recommended rate or as a test dose
- Infusion: Preferably administered by drip infusion in diluted solutions to reduce risk of hypotensive episodes
Interaction
Not studied; should not be administered concomitantly with oral iron preparations
Contraindications
- Evidence of iron overload
- Known hypersensitivity to iron sucrose or inactive components
- Anaemia not caused by iron deficiency
- History of allergic disorders, liver disease, and infections

Precautions & Warnings
- Monitoring of hematologic and haematinic parameters required
- Caution in presence of tissue iron overload
- Serious hypersensitivity reactions reported rarely
- Caution in administering to patients with hypotension
Use in Special Populations
- Pediatric use not established
- No significant differences observed in geriatric use
Overdose Effects
- Accumulation of iron in storage sites leading to hemosiderosis
- Symptoms: hypotension, headache, vomiting, nausea, dizziness, edema, cardiovascular collapse
- Monitoring of iron parameters for recognition of accumulation
- Avoiding iron overload by discontinuing when serum ferritin levels exceed guidelines
