Indications
- Hypertension
- Reducing hospitalizations in congestive heart failure
- Reducing death in patients with congestive heart failure post-myocardial infarction
Pharmacology
- Belongs to angiotensin receptor blockers (ARBs)
- Orally active
- Specific angiotensin II antagonist
- Acts on the AT1 subtype
- Blocks angiotensin II receptor
- Dilates blood vessels and reduces blood pressure
- No impact on pulse rate
- Greater affinity for AT1 receptor than AT2 receptor
- Does not bind or block other hormone receptors or ion channels
Dosage
- Usual dose for hypertension is 80 to 160 mg once daily
- Maximum dose is 320 mg daily
- Maximum blood pressure reduction within 4 weeks
- Usual dose for heart failure is 40 mg twice daily, may be increased to 80-160 mg twice daily
- Initial dose post-myocardial infarction is 20 mg twice daily, may increase to 160 mg daily if tolerated without side effects
Administration
- Take on empty stomach
- Decreased absorption by about 40% with food
- No initial dosage adjustment for elderly patients with mild to moderate renal and hepatic insufficiency
Interaction
- No drug interactions of clinical significance found
- Studied compounds include Cimetidine, Warfarin, Furosemide, Digoxin, Atenolol, Indomethacin, Hydrochlorothiazide, Amlodipine, Glibenclamide
- No expected metabolic induction or inhibition of the cytochrome P450 system
- Caution with co-administration of potassium sparing diuretics, potassium supplements, or salt substitutes containing potassium
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Rare side effects
- Common side effects include headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea
- Possible hyperkalemia, impotency, reduced renal function, allergic reactions, dyspnea, constipation, back pain, muscle cramps, rash, anxiety, insomnia, vertigo
- Possible hypotension with concurrent diuretic use
Pregnancy & Lactation
- Contraindicated in pregnancy after 1st trimester
- Potential adverse effects on the nursing infant
- Consideration for discontinuing nursing if taking the drug
Precautions & Warnings
- Exercise caution in patients with impaired hepatic function
- Dosage adjustment or discontinuation for patients with pre-existing renal impairment
- Caution in initiating therapy for heart failure and post-myocardial infarction patients

- No overall difference in efficacy or safety observed in geriatric patients

Overdose Effects
