Indications
- Reduction of myocardial infarction rate
- Reduction of stroke rate
- Unstable angina treatment
- Non-ST-elevation myocardial infarction (NSTEMI) treatment
- Recent myocardial infarction (MI) treatment
- Recent stroke treatment
- Established peripheral arterial disease treatment
Pharmacology
- Prodrug
- Irreversible binding to P2Y12 class of ADP receptors on platelets
- Dose-dependent inhibition of platelet aggregation
- Inhibition of platelet aggregation within 2 hours
- Steady state inhibition of platelet aggregation at 3-7 days
Dosage & Administration
- 300mg oral loading dose for acute coronary syndrome (ACS)
- 75mg once daily orally for recent MI, recent stroke, or established peripheral arterial disease
- Oral administration with or without food
Interaction
- Risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with omeprazole or esomeprazole
- Increased plasma concentrations of Repaglinide with concomitant use of Anplat and Repaglinide
Contraindications
- Hypersensitivity to the drug or its components
- Active pathological bleeding such as peptic ulcer or intracranial hemorrhage
Side Effects
- Common: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
- Consideration of drug importance to the mother when deciding to discontinue nursing or the drug
Precautions & Warnings
- Reduced antiplatelet activity in CYP2C19 poor metabolizers and with CYP2C19 inhibiting drugs
- Increased risk of bleeding due to lifetime platelet aggregation inhibition
- Increased risk of cardiovascular events upon discontinuation
- Reported Thrombotic Thrombocytopenic Purpura (TTP) requiring urgent treatment
- Reported hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment necessary in elderly patients

- Potential clotting ability restoration with platelet transfusion
