Pharmacology

  • Bactericidal action
  • Interferes with bacterial cell wall synthesis
  • Potent bactericidal activity against a broad spectrum of bacteria

Dosage

  • Adult dosage: 500 mg to 1 gm by intravenous administration every 8 hours
  • Children's dosage: 10 to 40 mg/kg intravenously every 8 hours
  • Dosage depends on type, severity of infection, and patient's condition

Administration

  • Intravenous infusion over approximately 15-30 minutes
  • Intravenous bolus over approximately 3-5 minutes

Interaction

  • Probenecid competition with active tubular secretion
  • Inhibition of renal excretion
  • Reduction of serum valproic acid levels

Contraindications

  • Hypersensitivity to R-Penem

Pregnancy & Lactation

Precautions & Warnings

  • Discontinue in case of allergic reaction
  • Careful monitoring in patients with hepatic disease

Use in Special Populations

  • Dosage adjustment for renal impairment
  • No dosage adjustment for hepatic impairment
  • No dosage adjustment for elderly unless creatinine clearance is <51 ml/min
  • Efficacy and tolerability not established in infants under 3 months

Overdose Effects

  • Accidental overdose in patients with renal impairment
  • Treatment should be symptomatic
  • Renal elimination and haemodialysis for removal of R-Penem
