Indications

• Treatment of chronic HCV genotype 1-6 infection
• Treatment with or without cirrhosis
• Treatment in pediatric patients 3 years and older

Pharmacology

• Sofosbuvir inhibits HCV NS5B RNA-dependent RNA polymerase
• Velpatasvir inhibits HCV NS5A protein
• Metabolism of sofosbuvir to form pharmacologically active uridine analog triphosphate

Dosage & Administration

• Recommended treatment regimen and duration based on patient population
• Recommended dosage in adults and pediatric patients
• Dosing for pediatric patients based on weight

Interaction

• Potential for other drugs to affect sofosbuvir & velpatasvir
• Potential for sofosbuvir & velpatasvir to affect other drugs
• Established and potentially significant drug interactions

Contraindications

• Contraindicated in patients for whom ribavirin is contraindicated

Side Effects

• Common side effects: fatigue, nausea, headache, anemia, diarrhea, insomnia
• Rare adverse events: reduced hemoglobin level, reduced lymphocyte count, serious bradycardia

Pregnancy & Lactation

• Contraindicated in pregnant women and men with pregnant female partners if administered with ribavirin

Precautions & Warnings

• Risk of hepatitis B virus reactivation
• Serious symptomatic bradycardia when coadministered with amiodarone

Use in Special Populations

• Effect on geriatric patients
• Effect on patients with renal or hepatic impairment

Overdose Effects

• No specific antidote available
• General supportive measures required
• Hemodialysis for removal of predominant metabolite of sofosbuvir

Therapeutic Class

• Hepatic viral infections (Hepatitis C)
