Indications

Dosage & Administration
- Individualized dose for mild to moderate hypertension
- Usual starting dose of 5 mg once daily
- Dose titration interval of 2 weeks
- Increasing the dose beyond 20 mg once daily produces small incremental benefit
Interaction
- Not to be combined with other β-blocking agents
- Monitoring needed with catecholamine-depleting drugs
- Caution with centrally active antihypertensive agents
- Use with care when myocardial depressants or inhibitors of A-V conduction are used concurrently
- Combined use with calcium channel blockers may lead to severe hypotension, bradycardia, and cardiac failure
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
- Sinus bradycardia

- Cold or numb extremities
- Muscle weakness or cramps
- Slower than normal heart

Pregnancy & Lactation
- Not teratogenic in rats at doses up to 150 mg/kg/day
- Fetotoxic at 50 mg/kg/day and maternotoxic at 150 mg/kg/day
- Not teratogenic in rabbits at doses up to 12.5 mg/kg/day
- Embryolethal at 12.5 mg/kg/day
- No studies in pregnant women
- Should be used during pregnancy only if potential benefit justifies potential risk to the fetus
- Small amounts detected in the milk of lactating rats
- Unknown excretion in human milk
Precautions & Warnings
- Use caution in adjusting the dose in patients with renal or hepatic impairment
- Patients with severe anaphylactic reaction history may be more reactive to allergens
- Unresponsive to usual doses of epinephrine used to treat allergic reactions
