Indications
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary disease with spastic bronchial component
Pharmacology
- Novel bronchodilator
- Structurally differs from Theophylline
- Selective inhibition of phosphodiesterase 4
- Relaxes bronchial smooth muscle
- Decreased affinities toward adenosine A1 and A2 receptors
- Inhibition of platelet activating factor (PAF) and generation of leukotriene production
Dosage & Administration
- Elderly: 200 mg tablet two or three times daily
- Adults: 400 mg tablet two or three times daily
- Children >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
- Children 6-12 years of age: 6-9 mg/kg body weight two times daily
- Doxophylline SR tablet for required daily dose of 400 mg
Interaction
- Avoid use with other xanthine preparations
- Limit consumption of caffeine-containing beverages and food
- Caution with ephedrine or other sympathomimetic drugs
- Potential drug interactions with erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, propanolol, phenytoin, and other anticonvulsants

- Known hypersensitivity to the drug or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- Lactation

Precautions & Warnings
- Factors reducing hepatic clearance of xanthine derivatives
- Caution in patients with cardiac disease, hypertension, severe hypoxemia, hyperthyroidism, liver disease, peptic ulcer, renal impairment
- Monitoring of blood levels recommended in certain cases
Overdose Effects
- Severe cardiac arrhythmias
- Tonic-clonic seizure
- Symptomatic treatment of cardiovascular collapse
- No specific antidote
