Indications
- Treatment or prevention of calcium depletion
- Associated conditions: hypoparathyroidism, achlorhydria, chronic diarrhea, vitamin D deficiency, steatorrhea, sprue, pregnancy, lactation, menopause, pancreatitis, renal failure, alkalosis, hyperphosphataemia
- Management of conditions associated with hyperacidity
- Fast relief of acid indigestion, heartburn, sour stomach, upset stomach
Pharmacology
- Reacts with gastric acid to produce a salt and water
- Postulated chemical reaction: CaCO3+2HCl = CaCl2+H2O+CO2
- Increase in gastric pH diminishes pepsin activity
- Up to 30% oral calcium load absorption
Dosage & Administration
- Recommended doses for adults: 540-2000 mg Calcium Carbonate per day
- Recommended doses for children: half of adult doses
- Dietary supplement for prevention of osteoporosis: 1250-3750 mg Calcium Carbonate per day
- Pregnancy and lactation recommended daily dose: 1200-1500 mg calcium
- Chronic renal failure doses: 2.5-9.0 gm Calcium Carbonate per day
- 1000 mg tablet dosage: 2000-3000 mg when symptoms occur; may be repeated hourly if needed
Interaction
- May enhance cardiac effects of digoxin and other cardiac glycosides in case of systemic hypercalcaemia
- May interfere with absorption of concomitantly administered tetracycline preparations
- Modification of vitamin D therapy may be required in chronic renal failure
Contraindications
- Hypercalcaemia and hyperparathyroidism
- Hypercalciuria and nephrolithiasis
- Zollinger-Ellison syndrome
- Concomitant digoxin therapy
- Cautious use in patients with sarcoidosis, renal or cardiac disease, and in patients receiving cardiac glycosides
Side Effects
- Orally administered may be irritating to the GI tract
- May cause constipation
- Rarely produces hypercalcaemia, especially in large doses in chronic renal failure
Pregnancy & Lactation
- Widely used in pregnancy for oral calcium supplementation or antacid therapy
- Safe for use in lactating women
Use in Special Populations
- Safe and effective in children and infants with chronic renal failure
- Elderly patients with renal failure may experience troublesome constipation
