Indications
- Hypertension
- Reduction of hospitalizations in patients with congestive heart failure
- Reduction of death in patients with congestive heart failure post-myocardial infarction
Pharmacology
- Belongs to angiotensin receptor blockers (ARBs) drug class
- Blocks AT1 subtype of angiotensin II receptors
- Dilates blood vessels and reduces blood pressure
- No effect on pulse rate
- High affinity for AT1 receptor over AT2 receptor
Dosage
- For hypertension: 80-160 mg once daily, maximum 320 mg daily
- For heart failure: 40 mg twice daily, may be increased to 80-160 mg twice daily
- Post-Myocardial Infarction: 20 mg twice daily, increased to 160 mg daily if tolerated
Administration
- Should be taken on an empty stomach
- Absorption reduced by 40% when taken with food
- No initial dosage adjustment required for elderly patients with mild to moderate renal and hepatic insufficiency
Interaction
- No clinically significant drug interactions found
- Studied compounds include Cimetidine, Warfarin, Furosemide, Digoxin, Atenolol, Indomethacin, Hydrochlorothiazide, Amlodipine, Glibenclamide
- Not metabolized significantly, no metabolic induction or inhibition expected
- Highly protein bound, caution with potassium sparing diuretics, potassium supplements, or salt substitutes containing potassium

Precautions & Warnings
- Care in patients with mild to moderate hepatic impairment including biliary obstructive disorder
- Dosage reduction or discontinuation may be required in patients with pre-existing renal impairment
- Caution when initiating therapy in heart failure and post-myocardial infarction patients
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall difference in efficacy or safety observed in geriatric patients
- Max dosage for mild to moderate hepatic impairment: 80 mg once daily, contraindicated in severe hepatic impairment
Overdose Effects
- Manifestations: hypotension and tachycardia
- Bradycardia could occur from parasympathetic (vagal) stimulation
- Treatment: supine position, intravenous infusion of normal saline if excessive hypotension
