  • Disseminated candidiasis
  • Prophylaxis for fungal infection in neutropenic cancer patients
  • Acute treatment of other systemic fungal infections such as coccidioidomycosis and histoplasmosis

Mode of Action

  • Potent inhibitor of fungal cytochrome P-450 dependent enzymes
  • Responsible for the synthesis of ergosterol

Dosage & Administration

  • 150 mg as a single dose for Vaginal Candidiasis
  • 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
  • 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
  • 150 mg weekly for 4-6 weeks for Tinea corporis/Tinea cruris/Tinea pedis/Other Tinea
  • 50 mg daily for 20 days for Kerion
  • 400 mg as a single dose for Pityriasis versicolor
  • 150 mg weekly for 12 months for Onychomycosis
  • Orally or by IV infusion, 400 mg on first day then 200-400 mg daily for Invasive candidal infections and cryptococcal infections (including meningitis)

Interaction

  • Increased prothrombin time after warfarin administration
  • Prolonged serum half-life of concomitantly administered oral sulphonylureas
  • Increased plasma concentrations of fluconazole with hydrochlorothiazide
  • Increased levels of phenytoin
  • Interaction with combined oral contraceptives
  • Slowly increased cyclosporin concentrations
  • No impairment of absorption with food, cimetidine, antacids, or following total body irradiation for bone marrow transplantation
  • Decreased mean plasma clearance of theophylline

Contraindications

  • Known hypersensitivity to Fluconazole or related triazole compounds

Pregnancy & Lactation

  • Adverse fetal effects seen in animals only at doses associated with maternal toxicity
  • Not recommended for use in pregnancy or in women of childbearing potential unless adequate contraception is employed
  • Found in human breast milk

Precautions & Warnings

  • Abnormalities in hepatic, renal, hematological and other biochemical function tests observed in some patients
  • Rare instances of hepatic necrosis
  • Rare exfoliative cutaneous reactions
  • Discontinuation in case of rash development
  • Not recommended during lactation
  • Unlikely to impair ability to drive or use machinery

Use in Special Populations

  • Normal dose recommended for the elderly with no evidence of renal impairment
  • No adjustments in single dose therapy for renal impairment
  • Normal dose for patients with creatinine clearance > 40
  • Dosage intervals/daily dose adjustment for patients with creatinine clearance 21-40, 10-20, or receiving regular haemodialysis

Overdose Effects

  • Supportive measures and symptomatic treatment
  • Gastric lavage if necessary
  • Forced volume diuresis may increase elimination rate
  • Haemodialysis decreases plasma levels by approximately 50%
