Description

  • Afluzole is an antifungal medication used to treat various fungal infections including vaginal candidiasis, oropharyngeal candidiasis, esophageal candidiasis, tinea corporis, tinea cruris, tinea pedis, kerion, pityriasis versicolor, onychomycosis, invasive candidal infections, cryptococcal infections, prevention of fungal infections in immunocompromised patients, and more. It works by inhibiting fungal cytochrome P-450 dependent enzymes essential for the synthesis of ergosterol, an important component of fungal cell membrane.

Dosage & Administration

  • Adults: Vaginal candidiasis - 150 mg as a single dose, Oropharyngeal candidiasis - 200 mg in 1st day followed by 100 mg daily for 14 days, Oesophageal candidiasis - 200 mg in 1st day followed by 100 mg daily for 14-30 days, Tinea corporis/cruris/pedis/other tinea - 150 mg weekly for 4-6 weeks, Kerion - 50 mg daily for 20 days, Pityriasis versicolor - 400 mg as a single dose, Onychomycosis - 150 mg weekly for 12 months, Invasive candidal and cryptococcal infections - 400 mg on first day then 200-400 mg daily, Prevention of cryptococcal meningitis - 200 mg daily, Prevention of fungal infections in immunocompromised patients - 50-400 mg daily, Children over 1 year: In superficial candidiasis - 1-2 mg/kg daily, In systemic candidiasis & cryptococcal infection: 3-6 mg/kg daily, with adjustments based on weight and age.
  • Use in Specific Population: Elderly patients, Renal Impairment, Children, and Lactating mothers require specific dosing considerations.

Interaction

  • Fluconazole may interact with warfarin, sulphonylureas, hydrochlorothiazide, phenytoin, rifampicin, combined oral contraceptives, cyclosporin, theophylline, cimetidine, antacids, and food.
  • Physicians should be aware that drug interaction studies with other medications have not been conducted, but such interactions may occur.

Contraindications

  • Fluconazole should not be used in patients with known hypersensitivity to fluconazole or related triazole compounds.

Side Effects

  • Common side effects of Afluzole include symptoms associated with the gastrointestinal tract such as nausea, abdominal discomfort, diarrhea, and flatulence. Other rare adverse events include rash and in rare cases, anaphylaxis has been reported.

Pregnancy & Lactation

  • Fluconazole adverse fetal effects have been observed in animals at dose levels associated with maternal toxicity. Its use in pregnancy or in women of childbearing potential is not recommended unless adequate contraception is employed. Its use during lactation is not recommended.

Precautions & Warnings

  • Patients, particularly those with serious underlying diseases like AIDS and cancer, may experience abnormalities of hepatic, renal, hematological, and other biochemical function tests during treatment.
  • Rarely, patients may develop exfoliative cutaneous reactions and severe cutaneous reactions like Stevens-Johnson Syndrome and toxic epidermal necrolysis.
  • Use in patients with liver enzyme rise should be assessed for risk-benefit ratio.
  • Use during lactation is not recommended.
  • Afluzole therapy is unlikely to impair a patient's ability to drive or use machinery.

Use in Special Populations

  • Use in the elderly: The normal dose should be used if there is no evidence of renal impairment. In patients with renal impairment, the dosage intervals or daily dosage should be adjusted based on creatinine clearance.
  • Use in renal impairment: Only minor adjustments are required in single dose therapy. In multiple dose therapy, adjustments in the dosage intervals or daily dosage should be made according to creatinine clearance.

Overdose Effects

  • In the event of overdosage, supportive measures and symptomatic treatment with gastric lavage if necessary may be adequate. Forced volume diuresis and haemodialysis can also be considered.
