Indications
- For hypertension
- To reduce hospitalizations in patients with congestive heart failure
- To reduce death in patients who developed congestive heart failure after myocardial infarction
Pharmacology
- Oral medication
- Angiotensin receptor blockers (ARBs)
- AT1 subtype antagonist
- Dilates blood vessels and reduces blood pressure
- Does not affect pulse rate
- High affinity for AT1 receptor
Dosage
- Hypertension: 80-160 mg once daily
- Maximum dose: 320 mg daily
- Heart failure: 40 mg twice daily, may be increased to 80-160 mg twice daily
- Post-Myocardial Infarction: 20 mg twice daily, increased to 160 mg daily if tolerated
Administration
- Should be taken on an empty stomach
- No initial dosage adjustment for elderly patients with mild to moderate renal and hepatic insufficiency
Interaction
- No clinically significant drug interactions found
- Highly protein bound
- Caution with potassium sparing diuretics or supplements
- No metabolic induction or inhibition expected

- Hypersensitivity to any component of the product

- Impaired Hepatic Function: Exercised caution in mild to moderate hepatic impairment
- Impaired Renal Function: Dosage reduction or discontinuation may be required
- Heart Failure and Myocardial Infarction: Caution when initiating therapy
Use in Special Populations
- Pediatric use: Safety and effectiveness not established
- Geriatric use: No overall difference in efficacy or safety observed
- Hepatic Impairment: Mild to moderate - Max: 80 mg once daily. Severe - Contraindicated.
Overdose Effects
- Likely manifestations: hypotension and tachycardia
- Treatment: place patient in supine position, intravenous infusion of normal saline if necessary
