Indications

  • Hypertension
  • Reduce hospitalizations in congestive heart failure
  • Reduce death after myocardial infarction with congestive heart failure

Pharmacology

  • Belongs to angiotensin receptor blockers (ARBs)
  • Blocks angiotensin II receptor
  • Dilates blood vessels
  • Reduces blood pressure
  • Orally active
  • Specific angiotensin II antagonist acting on the AT1 subtype
  • Does not bind or block other hormone receptors or ion channels

Dosage

  • Usual dose for hypertension: 80-160 mg once daily
  • Maximum dose for hypertension: 320 mg daily
  • Usual dose for heart failure: 40 mg twice daily, may increase to 80-160 mg twice daily
  • Usual dose post-myocardial infarction: 20 mg twice daily, may increase to 160 mg daily if tolerated

Administration

  • Should be taken on an empty stomach

Interaction

  • No clinically significant drug interactions found
  • No metabolic induction or inhibition of the cytochrome P450 system expected
  • Concomitant use of potassium sparing diuretics or salt substitutes containing potassium may lead to increase in serum potassium

  • Hypersensitivity to any component of the product

Precautions & Warnings

  • Exercised in patients with hepatic impairment including biliary obstructive disorder
  • Dosage reduction or discontinuation may be required with pre-existing renal impairment
  • Caution in initiating therapy in heart failure and post-myocardial infarction patients

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • No overall difference in efficacy or safety in geriatric patients observed

Overdose Effects

  • Manifestations may include hypotension, tachycardia, bradycardia
  • Patient should be placed in supine position, given intravenous infusion of normal saline if necessary
