Indications

Pharmacology
- Monosulphonyl diuretic
- Effective in low glomerular filtration rate
- Distinctive action on renal tubular function

- Inhibition of electrolyte reabsorption in the thick ascending limb of the loop of Henle

Dosage & Administration
- Available as Furosemide 40 mg Tablet
- Initial adult dose is 40 mg daily
- Children's dose varies by age group
- Usual initial dose for hypertension is 80 mg
- Dosage should be adjusted according to response
- Maximum daily dose should not be exceeded
- For advanced chronic renal failure, diuretic therapy may start with Furosemide
- Furosemide Syrup has a wide therapeutic range
- Furosemide Injection dosage varies by age group
Interaction
- Marked fall in blood pressure with ACE inhibitors
- Increased serum lithium levels when given concomitantly
- Increased toxic effects of nephrotoxic antibiotics
Contraindications
- Contraindicated in anuria
- Contraindicated in electrolyte deficiency
- Contraindicated in pre-comatose states associated with liver cirrhosis
- Hypersensitivity to furosemide or sulphonamides
Side Effects
- Disturbed electrolytes and water balance
- Potential for producing alkalosis
- Uric acid retention
- Potential for acute gout
- Potential to provoke hyperglycemia
- Potential to provoke glycosuria
Pregnancy & Lactation
- Pregnancy category C
- Caution in cardiogenic shock complicated by pulmonary oedema
- Monitoring of blood pressure and pulse during rapid diuresis
- Caution in patients liable to electrolyte deficiency
- Potential inhibition of lactation or passage into breast milk
Precautions & Warnings
- Increased risk of acute retention in patients with prostatic hypertrophy or impairment of micturition
- A marked fall in blood pressure may be seen with ACE inhibitors
- Increased toxic effects of nephrotoxic antibiotics
- Adverse effects may impair the ability to perform hazardous tasks
Overdose Effects
- Dependent on the extent and consequences of electrolyte and fluid loss
- E.g. hypovolaemia, dehydration, haemoconcentration, cardiac arrhythmias
- No specific antidote known
- Management may involve measures to limit further systemic absorption
